This card is accepted only a participating retail network pharmacies. When your eligibility terminates, this card is void. Any fraudulent or unauthorized use of this card is strictly prohibited by law. All medical decisions are between you and your healthcare provider. Member acknowledges that by acceptance of this card they have agreed to receive information by telephone or mail regarding diabetes testing supplies and other durable medical supplies offered by KingdomCare.
